Upgrade to Premium Membership

On the final iteration of this form, we want there to be a button for sign up they can register for free, and then a section that says “Upgrade to Premium Membership Now and Get Your First Month Free” and then put a button that says Upgrade Now. When they click the Upgrade button, a new form will appear to capture their billing information. The form below is a mockup of what will appear once they click that button.
